When to See a Specialist

Recognizing When Specialist Care Is Needed

Not every diabetic wound requires specialist intervention, but certain signs indicate that specialized wound care is needed. Early specialist involvement can prevent wounds from becoming chronic or leading to serious complications.

Wound Not Healing

If a wound hasn't shown significant improvement after 2–4 weeks of basic care, a wound care specialist should evaluate it.

Signs of Infection

Redness, warmth, swelling, increased pain, drainage, or fever — these signs warrant prompt specialist evaluation and treatment.

Deep or Large Wounds

Wounds that extend deep into tissue, involve a large area, or expose bone or tendon require specialist management.

Recurrent Ulcers

If ulcers keep recurring in the same location, a specialist can assess underlying causes — footwear, deformity, circulation — and develop a prevention plan.

Neuropathy with Calluses

Thick calluses over pressure points in patients with neuropathy can break down into ulcers. A specialist can provide preventive care.

Prior Amputation History

Patients who have had a previous toe or foot amputation are at higher risk for new wounds and benefit from specialist monitoring.

Specialist Treatment Options

What a Wound Care Specialist Offers

A wound care specialist has advanced training and tools beyond basic wound management. Cuba's program provides intensive specialist wound care with daily attention and Heberprot-P therapy — an option not available in the United States.

Advanced Debridement

Surgical removal of dead or infected tissue — including callus, necrotic tissue, and biofilm — to expose healthy tissue and promote healing.

Offloading Expertise

Custom total contact casts, removable cast boots, and specialized footwear prescribed and fitted by specialists.

Infection Management

Culture-directed antibiotic therapy, deep infection assessment, and coordination with infectious disease specialists when needed.

Heberprot-P Therapy

Cuba's intra-lesional rh-EGF therapy, administered directly into chronic wounds to stimulate healing. Not available in the US.

Vascular Coordination

Assessment of circulation and coordination with vascular surgeons for revascularization when blood flow is inadequate.

Individualized Care Plan

Our multidisciplinary team evaluates each patient individually and determines whether an international wound-care program may be appropriate.

Cuba's Approach

The Cuban Diabetic Foot Program

Cuba has developed a comprehensive diabetic foot treatment program centered on Heberprot-P — a recombinant human epidermal growth factor (rh-EGF) administered by intra-lesional injection directly into the wound. This therapy is combined with intensive, multidisciplinary wound care. Our team evaluates each patient individually and determines whether an international wound-care and limb-preservation program may be appropriate. Heberprot-P is not available in the United States.

  • Heberprot-P intra-lesional rh-EGF therapy — a Cuban biotechnology innovation
  • Intensive daily wound care by specialist teams
  • Multidisciplinary team: endocrinologists, podiatrists, vascular surgeons, wound care nurses
  • 4–8 week individualized program cycles with continuous monitoring
  • Limb preservation focus — our team evaluates each patient individually
